Fees/Payment:

I currently accept the following insurances: Husky, Anthem, and Medicare (application is pending).

My out of pocket fee is $200 per session. Reduced fees are available on a limited basis. Payment is expected at the time of service.  I accept all credit cards, checks, Venmo/Paypal and cash.

I can provide the documentation necessary to submit to your insurance company to get reimbursed through out of network benefits.

If electing to use out of network benefits, please ask the following questions of your insurance provider:

• What is my out of network deductible and has it been met?
  
• What is the out of network allowed amount per session?
  
• What percentage of that amount will I be reimbursed when I pay out of pocket?
  
• How many sessions per year does my health insurance cover?
  
• Is approval required from my primary care physician?
  
• How do I request reimbursement?
